In the arduous journey toward becoming physicians, medical students often encounter a multitude of challenges. In this episode, hosts Andy and John are joined by guests, Geoff Comp, DO and Megan Stobart-Gallagher, DO,  for a thoughtful discussion centered around the SOAP. SOAP, which stands for Supplemental Offer and Acceptance Program, provides a structured approach for candidates who didn’t match into their desired residency programs during the initial match process. This episode delves into the intricacies of the SOAP and how students can navigate this critical aspect of their professional journey.

Understanding the SOAP

SOAP, replacing the infamous post-match “scramble”, introduces a more organized approach to securing residency positions for medical students who find themselves without a match. Nevertheless, the process can be daunting, and understanding its complexities is essential. In the days following the initial matching process, programs with unfilled spots are tasked with compiling a new, comprehensive list of applicants that may be good fits for their program. In doing so, the aim for program directors is to find the best fit, while also maximizing the chances of a successful match.

Navigating the Complex Landscape of Residency Matching

After not initially matching, students receive emails indicating filled or unfilled residency positions. The subsequent preparation involves revising applications, cold calling, and emailing for interviews. To navigate this multifaceted landscape successfully, students can benefit from strategic planning, showcasing adaptability, and effectively communicating with programs to ensure the best outcome for both parties.

In addition to this, the SOAP introduces its own unique set of challenges that shape its dynamics. With restrictions on time and the types of communication that are allowed amongst programs and applicants, these intricate barriers underscore the complexity and constraints that must be traversed throughout the SOAP. In light of these intricacies, practical tips for navigating SOAP become invaluable, offering guidance on overcoming hurdles and optimizing the chances of securing a suitable residency position.

Practical Tips for Navigating the SOAP

Clear communication between program directors and applicants regarding expectations and selection criteria is of utmost importance during this seemingly strenuous process. While programs provide feedback and transparency, applicants should actively seek information, adapt to changes, and demonstrate flexibility in preferences. Embrace this as an opportunity to explore various specialties and programs that may not have initially been on your radar.

Taking proactive steps increases your chances of success and empowers you to face the SOAP  with confidence and an open mind. While not mandatory and not always possible, submitting a Standardized Letter of Evaluation (SLOE) from an EM program is one example of a way to communicate your competency in EM.

Emotional Realities: Coping Strategies for Challenges in SOAP

Grief initially accompanies the news of not matching, creating an emotional rollercoaster for medical students. Accompanied by this grief is the pressure, felt by both students and program directors alike, to match. Additionally, there may be additional pressures from, medical schools, family, friends, and even your own expectations. Dealing with information overload and the emotional complexities of SOAP is an inherent aspect of this journey. That said, it is essential for all parties involved to acknowledge that this challenging experience is universal. Both programs and applicants alike face uncertainties and disappointments.

So what can you as a student do? Dispel the notion that you are not worthy. You have made it this far along in your medical journey, and this is just another step along the way. Being honest with yourself is crucial during this process. Acknowledge your emotions and allow yourself to grieve, but know that this phase of SOAP is just a temporary challenge. The resilience you have demonstrated in reaching this point speaks volumes about your dedication and capabilities.

Take-Home Points:
  1.  Recognize that challenges are inherent for both programs and applicants within the SOAP.
  2. Prioritize open and transparent communication throughout the SOAP process to foster mutual understanding.
  3. Acknowledge your personal resilience and worth for being this far along in your medical journey!
  4. Embrace the notion that every step, even challenges in the SOAP, contributes to your journey with purpose and understanding.
